NURS FPX 6218 Assessment 2 Assessing Community Health Care Needs
Paper Instructions
Assess the health care needs of a selected community by completing an environmental analysis and a virtual windshield survey. Summarize the results of your assessment in a 2-3 page executive summary.
Jordan, Minnesota
Introduction
Health care must be evidence-based, effective, efficient, and affordable; it must provide resources that meet the needs of the community. Nurse leaders must understand and thoroughly evaluate the environment to enable the efficient and equitable allocation of resources. A useful tool for this type of assessment is commonly called a windshield survey. Windshield surveys are what you might expect from the name. They are a way of gathering information about specific aspects of a community while driving around, such as the condition of roads, buildings, and housing.
This assessment provides an opportunity to examine the prevailing health conditions and social determinants of health in a community by completing an environmental analysis and a virtual windshield survey based on communities within the Vila Health system. Although Vila Health is a virtual lab, the communities represented in this simulation are real, enabling you to conduct an actual community health assessment.
Preparation
Executive leaders at Vila Health have asked you to provide them with your assessment of the health conditions in one of the communities served by the Vila Health system. Knowing that a windshield survey is needed to validate any underlying assumptions about the needs of the community and inform evidence-based decision making and strategic planning, you have decided to conduct a first-hand exploration of the community, followed up by an environmental analysis. An environmental analysis examines the factors that can influence the performance of a health care organization, which is important in a rapidly changing health care environment.
To prepare for the assessment, you are encouraged to begin thinking about how an environmental analysis and windshield survey contribute to assessing community health care needs. In addition, you may wish to
- Review the assessment requirements and scoring guide to ensure that you understand the work you will be asked to complete.
- Review Guiding Questions Assessing Community Health Care Needs [DOCX], which includes questions to consider and additional guidance on how to successfully complete the assessment.
Note Remember that you can submit all or a portion of your draft assessment to Smarthinking Tutoring for feedback before you submit the final version for this assessment. If you plan on using this free service, be mindful of the turnaround time of 24-48 hours for receiving feedback.
The following resource is required to complete the assessment.
- Vila Health Environmental Analysis and Windshield Survey.
Vila Health is a virtual environment that simulates a real-world health care system. In the various Vila Health scenarios, you will apply professional strategies, practice skills, and build competencies that you can apply to your coursework and in your career. The information you gather in this scenario will help you to complete the assessment.

Executive Summary
General Condition and Public Health Needs
Community health assessment is always intended to find ways of enhancing and promoting health of a given community (Ganji et al., 2019). The current community health assessment will focus on the accessibility of medical health care for urgent health care needs in Jordan city in Minnesota. Windshield survey and environmental assessment was conducted in Jordan, which revealed that the city hosts approximately 61,000 people with sufficient expansion opportunities due to big space within the industrial park that covers more than 30 acres and nearly 200 acre piece of land that stretches outside the city’s bounds (Data USA, 2021). The vast rural area of the Jordan city is mostly attributed to the limitations in accessing urgent medical care. Therefore, this survey will put more emphasis in exploring the rural and towns around. One of the possible challenges during survey is poor road conditions and adverse weather conditions due to winter.
Following analysis of Vila Health Environmental Health and Windshield Survey, I was evident that Jordan is in dire need of mobile health clinic to cater for urgent health care needs. Currently, the city has only a small health clinic that is meant for seniors, adults, and pediatrics care. However, there is lack of facilities to provide urgent care. The nearest facility is about 20 minutes drive, which can be challenging to people living with disabilities, the elderly, and rural dwellers that often face challenges of transportation. The city is recording fastest growing community of young people, with impressive school district and great recreational facilities. As such, introducing mobile urgent care clinic would be essential in offering critical services such as management of chronic diseases, screenings, influenza shots, and bacterial cultures.
Mobile health clinics (MHCs) connote health care delivery model that strives to provide access to health care to rural dwellers and eradicate health disparities in vulnerable populations. They are credited with helping the patients to steer their way in seeking social needs and medical needs resources within the community, thus, enhancing the overall patient outcomes (Egwu, 2019). However, some of the potential challenges that may impede the success of MHCs include shortage of health care providers and legal issues since rules and regulations regarding licensure would be required to be reviewed to ensure that compliance among the physicians and other care providers working under the MHCs model.
Environmental Analysis
MHCs are crucial for vulnerable populations such as immigrants, homeless families, children, and under insured people. These vulnerable populations often encounter challenges in accessing health care due to costs, lack of insurance, language barriers, lack of transportation, and psychological problems. Finding ways to provide affordable care for these populations can greatly enhance patient outcomes and the entire community health (Egwu, 2019). The windshield survey and environmental analysis in Jordan showed the essence of enhancing health care accessibility within Jordan.
Consequently, MHCs will not only provide vulnerable populations and the inhabitants of Jordan access to health care but also ensure patient centered care. Essentially, there are many potential locations for MHCs in Jordan. However, the location of MHCs will be determined based on the information and observations of the windshield survey and environmental analysis. Eventually, the objective of the MHC is to provide quality, affordable, and safe health care to the people of Jordan, especially the vulnerable populations and rural dwellers.
Conclusions
MHCs can play essential roles in ensuring that the vulnerable populations and rural dwellers get quality, affordable, and safe health care services. Moreover, it provides opportunity for eradication of social and health disparities that are common in the current health care system (Carmack et al., 2017). Successful implementation of this proposed project will lead to quality, affordable, and safe health care to the people of Jordan, especially the vulnerable populations and rural dwellers (Malone et al., 2020). It will also foster referral to advanced health care facilities in complex cases. Ultimately, the residents of Jordan will record improved health care outcomes.
