This assessment provides an opportunity to propose a community health care change that is based on the community health assessment you completed in Assessment 2.
Preparation
Your community health assessment was well received by the executives at Vila Health. Consequently, they have asked you to recommend a change in the community that will improve health outcomes. They have also asked you to submit your recommendation in a formal change proposal. You are confident that you can present a strong case for change, including a financial plan, that will ensure the success of this initiative.
In this assessment, you will draft a change proposal to executive leaders to solicit support for a change to the current community health care system. To prepare for the assessment, you are encouraged to begin thinking about your justification for establishing the recommended change as a priority, a strategy for leading change, and how the change will be funded. In the assessment of the Jordan community in Minnesota, diabetes has been discussed as the major problem affecting a wide range of patients in the healthcare system.
The technological strategy will be one of the most effective measures that can aid in reducing the number of diabetic cases in the community. The leadership at the clinic may fail to provide the optimum support needed for this project. Strategies such as aligning the missions of the organization and increasing the nurse participating in the project will be adopted to address these potential barriers.
The other barrier that might be experienced in this process is the need for more interest among nurses operating within community hospitals. Factors such as reduced awareness of the program may contribute to this barrier (Mathieson et al., 2019). The community will need to be involved in the program to ensure that the intervention is implemented as desired. The successful implementation of this project will have a positive patient outcome, especially in addressing issues related to the high number of diabetic cases recorded in the past three years in the community.

Effective leadership is one of the aspects identified to affect the use of technology in managing diabetes in the organization. The leadership style focusing on meeting the desired patient safety would invest in better ways to incorporate modern technology in managing diseases (Kerr et al., 2020). As such, nurses and physicians in an organization need to incorporate better and more effective leadership that advocates the adoption of new technological strategies that meet their patient’s needs. The excellence in leadership has seen the organization have a high staff retention rate, innovation, and a highly motivated workforce. These elements define the increased ability of an organization to support change.
Interventions such as open communication and active involvement will be adopted to address the mentioned barriers. The training needs assessment process allows the organization to identify any gaps in its current training society and use the interest of the community in developing the desired change (Fernandez et al., 2019). These gaps should be analyzed, prioritized, and turned into the organization’s training objectives. The ultimate goal is to bridge the gap of poor diabetes management in the community.
It is also important to ensure that managers, trainers, and mentors deliver a consistent message during on-the-program training.
