Discussion Question

From the chapter The Changing United States Health Care System in your
course textbook, using either the story of Wald\’s efforts to improve the health
status of impoverished immigrant communities or the story of how advanced
practice registered nurses have struggled with their legitimacy within the health
care delivery system, compare your own experience in nursing.

  • Have there been challenges you have faced? If so, what strategies did
you employ to overcome these challenges?
  • How do you envision your career moving forward in advocating for your
    patients? Your community? Yourself?

Week 1 Discussion

Nurse practitioners play a unique role in the provision of evidence-based care in their healthcare settings. However, they experience a myriad of challenges that often threaten their autonomy in patient care. I have experienced a number of challenges in my practice.

One of them is the colleagues and patients comparing me with a doctor. Patients and other providers may not understand the differences between advanced practice registered nurses and doctors. I often address this challenge by explaining to them the differences between the roles and stressing the fact that I undertake a unique role that is different from those of the doctors.

The other challenge I have experienced in practice is not being able to practice as an independent practitioner. Different states have varying regulations on the practice by the advanced registered nurses. Advanced registered nurses practice independently in some states while other states require them to practice under physician supervision.

Such restrictions affect the autonomy of the nurse practitioners in patient care. I address this challenge by engaging in advocacy efforts that aim at eliminating practice barriers for advanced registered nurses (Buerhaus et al., 2017).

I envision a nursing career that will be characterized by the increased focus on the needs of the patients in the future. The nurses will be actively involved in ensuring the adoption of patient-centered models in patient care to optimize their care outcomes. I also envision a future where all the healthcare providers will prioritize the need for patient engagement and prioritization of their diverse needs.

I also envision a career that will be characterized by healthier communities due to their enhanced engagement in activities that promote their health. I envision myself being a nurse trained at the doctoral level in my specialty to enable me to provide highly specialized care to patients and personal and professional development to other nurses.
