Purpose To discuss scientifically tested cognitive-behavioral therapy (CBT) models, their evidence base, and treatment recommendations for individual disorders.
Two major clinical recommendations
- For patients with Panic disorder, the guideline recommend individual face-to-face mode of CBT with 10 and 15 weekly sessions; however, briefer intervention with 6–7 sessions is also shown to be effective.
- Individual CBT is recommended for patients with Generalized anxiety disorder comprising 12–15 sessions spaced closely (weekly) followed by more spaced sessions (once a month). CBT can be terminated either when the targets are achieved or when there is plateauing of response (Reddy et al., 2020).
Reflection of the Appraisal
The treatment recommendations were graded based on the Strength of Recommendation Taxonomy system, with three levels of recommendation, based on the quality, quantity, and consistency of evidence. The levels of evidence were graded as A, B, and C.
Grade A included meta-analysis of Randomized controlled trials (RCTs) with consistent findings or high quality individual RCTs. Grade B included systematic review or meta-analysis of lower quality clinical trials or studies with inconsistent findings or lower quality (Reddy et al., 2020).
Grade C included extrapolations from bench research, disease-oriented evidence, usual practice, opinion, and case series. The recommendations will guide me on how to employ CBT in patients with Anxiety and Obsessive compulsive disorders. I will also use to determine the mode of delivery of CBT such as individual, group, or internet-based therapy.
Clinical practice guidelines summarize the best available evidence in recommendations to guide clinicians in making the best clinical decisions. On the other hand, research studies involve generating new insights, testing hypotheses, and evaluating the safety and efficacy of interventions. The evidence from research studies is used to develop clinical practice guidelines.
