Assessment Description

The completed Individual Success Plan (ISP) signed off by the preceptor must be submitted to the digital classroom for faculty approval. Student expectations and instructions for completing the ISP document are provided on the “NRS-465 Individual Success Plan” document. Faculty must approve of the completed ISP. Obtaining preceptor feedback and signature is the responsibility of the student.

The Individual Success Plan is a clinical document that is necessary to meet clinical requirements for this course. Therefore, the form should be submitted with the preceptor’s hand-written signature. A typed electronic signature will not be accepted.

Written Capstone Project Change Proposal

  • Present a detailed written evidence-based capstone project change proposal utilizing elements developed in previous topics
    Incorporating a plan to evaluate the nursing intervention using in-depth evidence-based capstone project change proposal
  • Identify potential barriers in implementing the proposed capstone change project
    Illustrate inter-professional collaboration in the development of the change project proposal

Topic 9 Reflection Journal Identify important learning activities and events that happened during the course and the practicum experience

Presentation Development 

  • Develop a presentation of the capstone change proposal for an inter-professional group, leaders, and colleague
