In other words, the objective of the study is to determine whether the implementation of the case management in comparison to the traditional discharge planning can aid in the reduction of length of stay, emergency visits, and readmissions over the course of 6-12 months. In this evidence-based practice project, quantitative design will be applied to collect data.

There are several advantages to using a quantitative design in data collection. First, it allows one to measure specific variables and track changes over time.
