Discussion Question 1

The community health nurse plays several roles in collaborating with community stakeholders for population health promotion. One of the roles is as a health educator. The community health nurse educates the community stakeholders about health issues affecting them and the needed interventions. The nurse uses information from community diagnosis to prioritize the care needs of the community stakeholders (Hartzler et al., 2018).

The community health nurse also collects, analyzes data, and develops recommendations for the community stakeholders. The nurse uses data to inform the appropriateness of the adopted interventions. The community health nurse also plays the supervisory role in working with the community stakeholders. The nurse guides the adoption of healthy lifestyles and behaviors in the communities they serve. They also encourage the community stakeholders to play pivotal roles in creating safe communities for the vulnerable populations (Bigi & Bocci, 2017).

An example is educating the community members about the need for increasing access and affordability of healthy diets to prevent and manage issues including obesity and overweight. The community health nurse should appraise community resources as part of community assessment. The nurse encourages active stakeholder participation in activities that contribute to the health and wellbeing of its members. Appraising resources also increases the community members’ understanding of their role in health promotion and disease prevention.

The relevance of health promotion activities is also achieved by appraising community resources (Scott et al., 2018). For example, institutions’ involvement, including religious bodies, provides the nurse with insights into the religious aspects that can be incorporated into health promotion activities for optimum outcomes. Therefore, community resources are vital for health promotion.

Geopolitical and phonological place influences the context of community or population assessment and intervention. Accordingly, a geopolitical community comprises aspects such as community boundaries, structures, climate, and animals. The community’s geographical location may influence the health and wellbeing of the community members. For example, harsh environmental conditions, including drought, affect access to water and food, hence, the communities’ health and wellbeing of the communities.

Phenomenological place refers to the psychological centers and locations related to cultural values, beliefs, practices, history, education, and economics. Phenomenological aspects of a place, including economics, culture, and beliefs, may act as barriers to health (Peñafiel Palacios et al., 2021). For example, individuals from low-socioeconomic backgrounds often experience barriers to healthcare due to the unaffordability of care services.

Therefore, community health nurses utilize phenomenological place data to plan interventions to address barriers to care in vulnerable communities. The nursing process is utilized to identify healthcare issues and create an appropriate intervention, including screenings and referrals. The nursing process steps such as assessment provide information about the severity of health needs and required interventions, including referrals and further screening. They also utilize assessment data to develop nursing diagnoses and prioritize them accordingly (Ellis et al., 2020).

The planning stage enables nurses to determine the most appropriate interventions to optimize patient care outcomes. Interventions such as referrals and screening are adopted in the implementation phase of the nursing process (Ackley et al., 2019). Community health nurses also utilize the monitoring and evaluation phases to determine the effectiveness of the adopted interventions. Therefore, nurses utilize the nursing process to identify community and patient needs and interventions to achieve optimum health outcomes.
