Article Selection and Justification
Nurses observe many clinical problems that require evidence-based interventions. The search for evidence is usually intensive since nurses must select appropriate, current, and scholarly sources that explore a particular problem in detail. My topic is “effectiveness of case management for patients with chronic diseases.” The article I selected is “Experiences of case management with chronic illnesses a qualitative systematic review.” In the article, Yoo and Liu (2018) summarize evidence from other peer-reviewed articles about the use of case management in chronic disease management.
On how the article relates to my potential DNP project topic, Yoo and Liu (18) explain what case management entails and its benefits when used for patients with chronic diseases. Case management is described as a collaborative process of assessing, planning and facilitating care coordination to meet an individual’s comprehensive health needs. The care process is coordinated through communication, engaging the family, and using the available resources. Regarding the benefits, case management helps achieve patient-centered care, improve care quality, and lower costs for individuals with chronic illnesses.
The main search terms used in the search process include case management, chronic diseases, effectiveness, benefits, and readmissions. The search terms were combined to form such phrases, including “effectiveness of case management for patients with chronic diseases,” “case management and diabetes,” and case management, chronic diseases, and readmissions.” Filters included publication dates (2018-2022) and peer-reviewed to ensure that all articles are appropriate for nursing research.
When reviewing the search results, I scanned through the abstract to check the article’s main study. Boolean article “and” was used to combine search terms and get appropriate phrases. Articles exploring case management but not on chronic diseases were considered inappropriate.
